What is LL-37?

LL-37 5mg is a germ-fighting peptide of 37 amino acids that the body makes for itself. Laboratories have studied it closely, and two small human trials have tested an LL-37 product.

Humans make just one cathelicidin, a family of germ-fighting peptides, and LL-37 is it. It is released by immune cells and by the cells that line the body's surfaces.

In lab work it attacks bacteria head-on and also talks to the immune system, which keeps researchers busy.

What the 5 PubMed papers cited for LL-37 looked at, 2014 to 2024

According to a 2016 review of lab studies, LL-37 tears holes in bacteria and alerts the immune system, and it can push inflammation either way.

Another review from the same year covered LL-37 research in the immune system, the lungs, the gut and the skin, and asked for more study.

Leg ulcers that would not close were the test in two randomised trials of an LL-37 product: one in 2014 with 34 patients, and a larger one in 2021 with 148 patients, against placebo. Both measured how the wounds closed, and the larger 2021 trial found no overall difference from placebo.

A 2024 skin review adds two cautions: peptides of this kind have often fallen short in clinical trials, and some bacteria resist them.

Reading the LL-37 record

LL-37 shows how much lab work can sit behind a small human record. In the lab it breaks open bacteria and calls in immune cells. In people, the record is two leg ulcer trials of one product.

The two trials differ in size: the 2021 trial took in 148 people, more than four times the 34 in the 2014 one. Reading one without the other would give half the record.

A 2016 review notes that LL-37 can push inflammation either way. The 2024 review adds that many peptides like it have fallen short in clinical trials, and that some bacteria resist them.

The 2024 review is worth reading in full for anyone weighing this peptide. It sets out both the lab work and the reasons such peptides have struggled in clinical trials.

Questions about LL-37

Does the human body make LL-37?

It does. LL-37 is the single cathelicidin humans produce, and immune and lining cells release it to fight germs.

How were the LL-37 ulcer trials run?

Both were randomised trials of an LL-37 product on leg ulcers that would not close: 34 patients in 2014 and 148 in 2021, against placebo. The 2021 trial found no overall difference from placebo.

Do bacteria ever resist LL-37?

A 2024 skin review says some do.
