What is SS-31?
SS-31 10mg, whose drug name is elamipretide, targets mitochondria: it is a peptide just four amino acids long.
The US Food and Drug Administration gave elamipretide accelerated approval in September 2025 for muscle strength in Barth syndrome, a rare inherited disease, as the PubMed record Elamipretide: First Approval (Drugs, 2026) reports. This research-grade vial is a different thing from that medicine, and no approval covers its use in people.
Its anchor is cardiolipin, a fat found in the inner membrane of mitochondria, which make the cell's energy.
That same fat is damaged in Barth syndrome, an inherited condition that is rare, and the 2025 approval covers only muscle strength in it.
What the 4 PubMed papers cited for SS-31 looked at, 2020 to 2026
Animal work on elamipretide, in heart, kidney, brain and muscle, was gathered with its human trials in a 2025 review.
Those human trials are small. A 2020 crossover trial of 30 adults whose muscles were affected by a mitochondrial disease measured a walking test and tiredness, on elamipretide and on placebo. It found no clear gain on the walking test.
The 2021 Barth syndrome trial, a phase 2/3 study of 12 patients, began as a randomised, double-blind, placebo-controlled crossover. Its main measures were a 6-minute walk test and a Barth syndrome symptom scale, with knee muscle strength, the patients' own view of their symptoms and some heart measures beside them. In that placebo-controlled part, neither main outcome was met. An open phase with no placebo group followed, which 10 people joined and 8 completed to 36 weeks. A 2026 drug profile then recorded the American approval of 2025.
Outside Barth syndrome the human trials are few and small, and the open stage of the Barth trial had no placebo group to compare against.

Reading the SS-31 record
